Choice Theory Reality Therapy (CTRT)

Choice Theory Reality Therapy (CTRT) is a counselling approach developed by psychiatrist William Glasser. It focuses on how people understand their needs, relationships, and choices in everyday life.

The approach proposes that much of human behaviour is influenced by attempts to meet several core psychological needs. When these needs are not being met in satisfying ways, tension may begin to appear in relationships, work, or personal decisions.

Reality Therapy explores how present actions relate to these needs and how different responses may influence the direction of one’s life.

Key ideas in Choice Theory

Choice Theory proposes that several psychological needs play an important role in human motivation. These include:

connection and belonging

a sense of competence or achievement

autonomy and personal choice

enjoyment and engagement in life

physical wellbeing and safety

When these needs are not being met in satisfying ways, people may notice recurring tensions in relationships or decisions. Exploring these needs can sometimes help clarify what someone is wanting from important areas of life and relationships.

When people often explore this approach

This framework may be helpful when someone is exploring situations such as:

  • ongoing difficulties in important relationships
  • feeling pulled between competing responsibilities or expectations
  • repeating behaviours that do not lead to the outcomes they hope for
  • uncertainty about how to move forward in certain areas of life

The focus of the work is on understanding how present choices relate to what someone wants from their relationships and their life.

How Reality Therapy works in counselling

Reality Therapy focuses primarily on present behaviour and decision-making.

In counselling this often includes:

  • clarifying what someone wants in important areas of life
  • examining how current actions are influencing those goals
  • considering alternative responses or choices that may move situations in a different direction

The process is collaborative and practical, with attention placed on situations that are currently unfolding in a person’s life.

How CTRT is used in counselling

Reality Therapy focuses on present choices and current relationships.

Rather than analysing past events in detail, the work centres on what someone wants now, what they are currently doing, and whether those actions are helping them move toward the life they want.

The approach emphasises personal choice while recognising the real constraints people may face in their lives.
